Delirium is a Lem + Ist + Io runeword created in 3-socket helms. While it provides interesting effects like transforming into Bone Fetish, it's rarely used in serious builds due to loss of character control during transformation. Primarily a meme/fun runeword rather than endgame optimization.

The Delirium Transform: When you're struck, 1% chance to transform into Bone Fetish for 93 seconds. During this time:
Cannot use skills (character only basic attacks)
+50% run speed
+261 defense
Confuse nearby enemies
Why It's Problematic: Loss of skill usage for 93 seconds is crippling. Most builds rely on active skills for damage/survival. Being forced to basic attack for 1.5 minutes makes Delirium a liability in difficult content.
MF Horker Barbarian: Barbarians using Find Item don't need skills active while hork-ing corpses. Delirium's 50% GF and 25% MF help, and transformation timer can be waited out between mob packs.
Summon Necromancer (Niche): Necro summoners rely on pets for damage, not active casting. Delirium transformation doesn't affect pet AI. The +2 skills boost summons. However, you lose ability to re-summon or use Corpse Explosion during transform.
Novelty/Fun Builds: Some players use Delirium for the absurdity factor. Transforming into Bone Fetish mid-combat is amusing, and the confusion effect on enemies creates chaos. Not competitive but entertaining.
Why Tiara? Tiaras are elite helms that can roll +3 to skill tree (Sorceress, Necromancer, etc.). Using Delirium in rare tiara with +skills wastes the tiara's potential, but provides highest possible defense.
Alternative Bases:
Demonhead (high defense, looks cool)
Bone Visage (high defense elite option)
Any 3-socket helm works (use cheapest bases since Delirium isn't serious endgame)
Socket Quest: Larzuk quest on Tiara gives 2-3 sockets (random). For guaranteed 3 sockets, use white Demonhead or Bone Visage (always get max sockets from Larzuk).
Novelty Factor: Some players want to experience every runeword. Buying pre-made Delirium lets you test it without wasting your own Ist rune.
Collection Completion: Runeword collectors need every runeword made. Purchasing saves the Ist + Lem cost for collection purposes.
Early Ladder Economy: In first week of ladder, Delirium provides +2 skills cheaply compared to Harlequin Crest which costs Ist-Gul early on. Viable temporary helm while farming for Shako.
No. Once Delirium triggers, you are stuck in the Bone Fetish form for the full 93 seconds. The transformation cannot be canceled, dispelled, or interrupted.
Yes. All skill bonuses remain active during the transformation. Existing summons and pets keep the bonus levels, but you cannot cast new skills while transformed.
Not really. While Delirium provides 25% Magic Find and 50% Gold Find, Harlequin Crest offers 50% Magic Find without the dangerous transformation drawback. Shako is generally the better choice.
No. Triggering the transformation during a duel effectively results in instant death, as you cannot cast spells or react while your opponent attacks freely.
Paladin auras remain active. Even while transformed, Paladins continue to provide aura effects such as Holy Freeze or Conviction to themselves and party members.
No. It is a very poor choice for Ubers. Uber bosses can easily kill a transformed character, especially since you cannot cast Life Tap, reposition, or resummon allies.
Yes. Mercenaries can use Delirium, and the transformation is less disruptive since mercs do not cast skills. They continue attacking while transformed, though it is still not an optimal helm choice.
