The Maggot King is a boss unlocked after completing the grandmaster quest The Blood Moon Rising. This boss is a brand new solo encounter that can reward you with the new best-in-slot ranged amulet, the Rupture Necklace, and the new best crush special attack weapon, Crimson Kristen.
In this complete Maggot King boss guide, we cover how to get there, the minimum and recommended stats, the best gear setups from budget to best-in-slot, and a full breakdown of the punish mechanic. We also cover the full drop table.
Fastest Way to Reach The Maggot King
To gain access to the Maggot King, you must first complete the grandmaster quest The Blood Moon Rising, which has the hardest quest boss in the game, with 3000 health and a bunch of new mechanics that players need to learn. Thankfully, we have a complete Lord Drakan Final Boss Guide on our blog that breaks down his mechanics and teaches you how to kill him if you are struggling.
After finishing the quest, you can use your Drakan’s Medallion to teleport to Castle Drakan, where you will find a portal to enter the Vampyrium. Once in, grab a weapon and head to this location shown below:

Here you will find a hooded figure being attacked by some monsters. Get rid of the monsters to save him. After saving him, the NPC will thank you and appear near the bank chest. You can follow this NPC to quickly reach the forest area of the map.

After following him, you can follow the path North and use any Agility shortcuts along the way to reach the Maggot King's Lair.

Maggot King Mechanics
Magic and Ranged Attacks
Maggot King has two standard attack being, Magic and Ranged. Magic attack is indicated by it being yellow, and Ranged attack is indicated by it being green. Both of the attacks split into three projectiles after it hits the player character and fall on the nearby tiles, creating pools.
- Green pools deal poison damage and reduce your poison immunity by 45 seconds if you step on them.
- Yellow pools will delay your next attack by a tick if stepped on, and they will lower a random combat stat.
You can always dodge these pools by moving 2 tiles after each hit.
You can completely ignore the poison pools if you are using both a serpentine helmet and an anti-venom+. You can also completely ignore yellow pools if you have an inoculation bracelet equipped.
The boss will always start with ranged attacks, and it will alternate between styles after every screech. It won’t ever use the same style in a row.
Screech
In between attack styles, the boss will stop and scream out. If you have an active overhead prayer at the time of the screech, you will take a good chunk of damage, so make sure to turn off your overhead prayer.
Carrion and Larvae
With every screech, Maggot King will also throw down some Carrion onto random tiles over the arena. Just like the attacks, once they hit the ground, they split and scatter around. This attack is telegraphed by shadows appearing over the tiles that the carrion will fall on; simply avoid these tiles to not take any damage.
After reaching 1250 health, the boss will also spawn some larvae alongside this attack. These larvae move in a straight line and eat up any carrion on the ground, which then heals the boss. If a larva is on the same tile as you, it will start doing rapid damage to you. There are two ways to deal with this:
- You can equip a bow such as a Craw’s bow or Venator’s bow to shoot the larva while they are in the air. This will always one-shot them, and the shot can be fired instantly. Venator bow is great since it can kill all 4 larvae the boss spawns towards the end of the fight.
- You can ignore the larvae as they are not that threatening and their healing is insignificant. If you are doing this, just make sure not to stand on top of one.
Melee Attacks and Punish Mechanic
Maggot King will start walking up close and slamming its head in a 3x5 AoE in front of it. This attack must be avoided at all costs as it deals around 35-50 damage if you get hit. Once the boss transitions to this melee attack, you must equip all your melee gear and start doing the punish mechanic. The boss will always move where your character is first before slamming its head. You can turn off your overhead prayers for this phase. There are two ways to do the punishment mechanic:
Lazy Way
If you don’t care about speeding up your kills and just want to have the easiest experience, this is the method for you. For this, you don’t need 2 melee weapons; instead, just bring one strong melee weapon like an elder maul. Simply walk up to the boss and, as it is slamming, walk a tile to his side to avoid the attack, and on the same tick click on the boss to do an instant melee attack. This is basically your character trading blows with the boss; it doesn’t provide the best DPS, but it is the easiest method.
Dodging to the side as shown below:

And one ticking him:

DPS Way
For the DPS way, you will need two weapons: a slow but heavy-hitting weapon and a 4-tick weapon. For this method, instead of waiting for the boss to move to your character, you are going to step in and out of the boss's attack radius to keep him from moving and triggering the 0-delay punish mechanic constantly.
Use your inquisitor’s mace or any other 4-tick weapon when you are inside the attack radius and use your elder maul when you step outside of it.
Using mace and stepping out on the same tick:

Using 0-delay melee punish and stepping back in:

Maggot King Full Fight Breakdown
Here is a step-by-step guide for defeating the Maggot King:
- Use your saturated heart, divine super combat potion, put on protection from Ranged and augury, and enter the boss lair.
- Start hitting the boss with fire spells. After every hit you receive, move two tiles to any side you want to dodge pools.
- Once the boss stops to screech, turn off your protection prayer while also dodging the incoming carrions from the ceiling. Once the screeching is over, turn on your Protect from Magic prayer. Keep alternating between the two whenever the boss screeches.
- If the boss is below 1250 health and you brought a bow along with you, you can shoot the incoming larvae in the air whenever the boss screeches. If not, simply ignore the larvae. If you are ignoring the larvae, it is wise to disable the attack option on them to walk more easily. Make sure you never stand on one to avoid taking damage.
- Usually, below 1000 health, the boss will start doing melee attacks after a screech. Equip your melee weapon and armor and start hitting the boss every time it tries to hit you. You can use the “Lazy Way” or the “DPS Way” discussed above. Either way, make sure to dodge this attack to not receive 35-50 damage.
- Keep alternating between styles and prayers according to the boss, and when the boss reaches around 300 health, it will screech and keep meleeing you until it dies. Keep doing the melee punish rotation to finish the fight.
- Magot King drops a lot of supplies. The dull ancient medal can be used to restore prayer, while stymphike tartares can be used to heal. With the addition of using an amulet of blood fury, you can keep repeating this fight and get 1-2 minute kills.
